Common Base Station Operator (Service Provider) licence application form
This application form provides details about Private Business Radio (PBR). The different Business Radio licences allow the licensee to operate different types of systems in accordance with licence conditions and Wireless Telegraphy legislation.
1. Notes
Please print out attached below form IN FULL and return it to the relevant address given in Section 2.
Please do not buy any business radio equipment until you have been assigned a frequency - otherwise you may find that the equipment is not compatible with the frequency you are assigned.
Please write clearly in black ink, using CAPITAL LETTERS.
Please remember to sign the declaration at the end of the form.
Please keep a copy of your completed application form.
If you are applying for a new licence or to take over an existing licence, please enclose the correct licence fee with your application. The fee depends on the frequency band used, and in some cases on the area of operation. For details of fees, visit the Ofcom website or call the Ofcom Contact Centre.
