This form is used to provide
comments on documents that are subject to a formal review. Click to download

Instructions for use: This
form is protected to prevent any manipulation of the form's layout. To
use this form, place the cursor in the first field, next to the box entitled
RA Document Title, and fill in all relevant boxes, pressing the tab
key to take you to the next field. In the Comment Type column,
you will find a pull-down menu. Select the comment identifier you require,
and tab to the next field as normal. If you require more space for
comments, simply download another copy of the form from this webpage.
Should you wish to request a review of any document relating
to Fixed Links, you should fill out a Document Issue Report. The document
can then be reviewed within the Fixed Links Consultative Committees. This
form is intentionally not protected, so the comments space may be expanded as
required.
